1. Parental Authority
By completing this registration, you confirm that:
You are the parent or legal guardian of the child being registered.
You have legal authority to provide consent.
All information provided is accurate and complete.
2.Health Declaration
You confirm that, to the best of your knowledge, your child:
Is physically fit to participate in martial arts training.
Has no medical condition that would make participation unsafe unless disclosed.
Has not been advised by a doctor to avoid physical activity.

You agree to keep us updated of any changes to your child’s health. If any health concerns exist, you agree to seek appropriate medical advice before your child participates.
3. Assumption of Risk
You understand that martial arts is a contact sport and involves inherent risks, including:
Bruising, bumps, and minor injuries
Sprains, strains, or fractures
Concussion or more serious injury in rare cases
While we take reasonable steps to provide a safe environment, injuries can occur. You voluntarily accept and assume all risks associated with your child’s participation.
4. Liability
Lions Cub Gym Ltd, its directors, instructors, staff, and agents will take reasonable care to provide a safe environment.
Participation is at your own risk. Lions Cub Gym Ltd shall not be liable for injury, illness, or loss arising from participation except where caused by negligence.
Nothing in this agreement excludes or limits liability for death or personal injury caused by negligence, or any liability which cannot legally be excluded.
5. Medical Treatment
In the event of an emergency, you authorise staff to seek appropriate medical treatment for your child if necessary.
6. Photography & Media
Lions Cub Gym Ltd may take photographs or video recordings during classes, gradings, or events for use on:
Website
Social media
Promotional materials
Coaching and training purposes
All media will be used respectfully and in line with safeguarding standards.
If you do not wish for your child to appear in photographs or videos, you must notify us in writing. We will record your request and ensure your child is not included in future media.
Where a media opt-out request is received, your child may be issued with a wristband or identifier to wear during classes to assist staff in ensuring they are not included in photographs or video recordings.
7. Parental Agreement
By signing below, you confirm that:
You have read and understood this agreement.
The information provided is accurate.
You accept the risks associated with martial arts training.
You give permission for your child to participate in classes at Lions Cub Gym Ltd.
